Instructions
- Select your form provider (I’m using Fluent Forms)
- Create your form and copy your form shortcode with whichever form provider you are using
- Navigate to Settings > WP Care Plan
- Open “Form Settings”
- Check the “Display Form?” checkbox
- Enter in a title for your form
- Select “Form Shortcode” from the Form Type
- Paste the shortcode in the last field
- Click “Save Settings”
Please note: Formidable Forms, Ninja Forms and Metform are not compatible with the WP Care Plan Plugin.
