How to Embed a Google Form

Instructions

1. Navigate to Google Forms and create or select the form you wish to embed
2. With your finished form, click “Send”
3. Click the last icon in the “Send via” row (the code brackets)
4. Copy the Embed HTML that appears by clicking the “Copy” button
5. Navigate back to WordPress and Settings > WP Care Plan
6. Open up the “Form Settings” section
7. Check the checkbox next to “Display Form?”
8. Leave the Form Title empty/blank (Google adds the title for you)
9. Select “Google Form” from the Form Type select
10. Paste the iframe code you copied in step 4 above
11. Click “Save Changes”

Note: You can also use this field to use other third-party form providers that provide iframe embed codes.
